Gynecologic and obstetric history
Menarche, cycle, LMP, obstetric formula (G P C A E LB) with a consistency check, contraception, menopause and HPV vaccination.
Exam and procedures
Breasts, abdomen, external genitalia, speculum and bimanual exam with your own normal baseline. Pap, HPV, IUD, implant, biopsy and colposcopy.
Results with Bethesda and BI-RADS
Pap smear with Bethesda 2014, HPV (16/18 or other), mammography with BI-RADS, and biopsy. An abnormal result cannot be closed as a routine follow-up.
Screening plan per patient
Interval in months for Pap and mammography. The first mammogram is scheduled based on the age the clinic sets.
Pregnancy with GA and due date
Due date by Naegele’s rule or early ultrasound, with a suggested correction. GA in weeks+days, ultrasounds, prenatal labs and a WHO or classic visit schedule.
Prenatal visits
Fundal height, FHR, fetal movements, edema and presentation, with flags for BP ≥ 140/90, FHR outside 110–160 and weight gain by pre-pregnancy BMI.
What happens on its own
- When you note a Pap, HPV test or biopsy in the visit, the sample is tracked as pending with no extra steps.
- If the result does not arrive within the clinic’s time limit, the front desk gets a task to follow up with the lab.
- When a patient is due for screening, she gets a reminder to book, with her consent and outside quiet hours, and the front desk sees the task.
- Each prenatal visit leaves a task to book the next one according to the chosen schedule.
- With an active pregnancy or breastfeeding, the prescription flags drugs to avoid according to the drug reference.
