Therapy processes, scales and risk follow-up, with private notes
You organize each therapy process with its reason, goals, frequency and informed consent. The system counts sessions, scores the scales with the published cutoffs and alerts you when risk is high; the interpretation is yours.

Therapy process
Individual, couple or family, with reason, goals, approach, weekly, biweekly or monthly frequency and planned sessions. Requires a guardian if anyone is under 18.
Session record
Attendance, in-person or online modality, topic, mental status, intervention, homework and risk level inside the session note.
Private notes
Notes only their author can see. The front desk and other professionals in the clinic have no access to them.
PHQ-9 and GAD-7
The patient answers from a link, or you enter the answers. The score is added up, the severity band is assigned and you see the trend in a chart.
Risk level
None, low, moderate or high, with notes. High risk leaves a flag on the chart and a 48-hour follow-up task for the therapist who runs the process.
Closing the process
Discharge, dropout, referral or another reason, with a closing summary kept in the record.
What happens on its own
- Before the session, the patient gets the link to the scales on the chosen schedule (first session, every 4 or every session), valid for 48 hours.
- Any answer other than “not at all” on PHQ-9 item 9 creates a risk follow-up task.
- High risk marked in the session leaves a 48-hour follow-up task.
- If a patient with moderate or high risk misses a session, the front desk sees the no-show flagged for a priority call.
What the patient receives
- Secure link to complete the PHQ-9 and GAD-7
- Pending scales in the patient portal
